Please email Corrie at corrie@livingwellvt.com with your full name, phone number, and insurance company name, and she will send you the paperwork and will set up your new patient appointment portal online. If you would prefer to call us for your new patient appointment, you can do that too at our main office number 802-658-6092.

Please note: Paperwork is a requirement and needs to be filled out prior to your appointment. If paperwork is not filled out before your appointment, your treatment time will be shorter for you to complete the paperwork.
